Terms & Conditions
The Yoga Therapy Clinic will provide a Service Agreement between Yoga Therapy Clinic and the participant.
This Service Agreement is made for the purpose of providing supports under the Participant's National Disability Insurance Scheme (NDIS) plan.
A copy of the Participant's NDIS plan is to attached to this Service Agreement by the participant.
The Parties agree that this Service Agreement is made in the context of the NDIS, which is a scheme that aims to:
support the independence and social and economic participation of people with disability, and
enable people with a disability to exercise choice and control in the pursuit of their goals and the planning and delivery of their supports.
The Provider, Yoga Therapy Clinic, agrees to provide:
Initial Consultation Session:
NDIS Quarterly Review:
Where the Yoga Therapy Clinic provides a home mobile service, travels costs of up to a maximum of 30 minutes each way will be claimed at the hourly rate when in an MM1 ZONE (metropolitan)
Where the Yoga Therapy Clinic provides a home mobile service, travels costs of up to a maximum of 30 minutes each way will be claimed at the hourly rate when in an MM2 ZONE (regional)
Cancellations Of Support Sessions
Where Yoga therapy clinic has a short notice cancellation (or no show) YTC will recover 90% of the fee associated with the activity,
A cancellation is a short notice cancellation if the participant:
does not show up for a scheduled support within a reasonable time, or is not present at the agreed place and within a reasonable time when the yoga therapy clinic is travelling to deliver the support; or
has given less than two (2) clear business days’ notice to cancel a support appointment.
Responsibilities Of The Participant (or representative if applicable)
The Participant agrees it is their responsibility to:
give yoga therapy clinic the required notice if they cannot attend a scheduled appointment, noting that if the notice is not provided, yoga therapy clinic cancellation policy will apply;
give Yoga Therapy Clinic the required notice if the client needs to end their Service Agreement and let Yoga Therapy Clinic know immediately if the client NDIS plan is suspended or replaced by a new NDIS plan, or the client stops being a participant in the NDIS.
The Provider will seek payment for their provision of supports after the Participant received delivery.
PLAN NOMINEE MANAGED If the funding for any of the supports provided under the Service Agreement is managed by a Plan Nominee:
The Participant's Nominee manages the funding for supports provided under this Service Agreement. After providing those supports, the Provider will send the Participant's Nominee an invoice for those supports for the Participant's Nominee to pay.
The Participant's Nominee will pay the invoice by EFT within 5 days.
The Participant will nominate the Plan Management Provider to manage the funding for NDIS supports provided. After providing those supports, the Provider will claim payment for those supports from the plan management provider. advised.
All invoices from Yoga Therapy Clinic are to be emailed to accounts department of the plan management provider.
